CloudMD is in search of dedicated bilingual (French & English) medical professionals to join our Healthcare Navigation team as a Nurse Care Coordinator (NCC). Full fluency in French and English (oral and written) are a must.

Position Summary

The Nurse Care Coordinator (NCC) serves as a trusted advisor and primary point of contact for intake and triage, playing a vital role in delivering seamless, high-quality care. In this critical frontline position, the NCC ensures effective communication and supports clients by conducting telephonic and digital assessments, navigating them toward the appropriate services. The NCC connects Employee and Family Assistance Program (EFAP), Student Assistance Program (SAP), and Telemedicine clients to the care that best meets their unique needs.

We are seeking Registered Practical Nurses (RPNs) with broad healthcare experience to address a range of concerns including physical health, mental health, stress management, chronic disease, eldercare, and disability-related matters. This role demands exceptional clinical judgment, organization, and empathy to enhance both client care and satisfaction.

The ideal candidate will demonstrate influence, compassion, and a commitment to ensuring every client receives the most effective and appropriate care, contributing to the overall success of the organization.

What you’ll be doing

  • Complete clinical intake assessments via telephone, chat, or through digital platforms to capture all relevant information on members' current conditions and health history.
  • Assess and refer members to the most appropriate services, programs or clinical counselors based on their presenting concerns
  • Work closely with CloudMD’s diverse stakeholder group including both mental and physical health clinicians, therapists, Nurse Practitioners, and other business units
  • Provide health coaching and health care navigation to members, while creating care plans and health goals
  • Ensure clear and thorough documentation on assessment and case management documentation for each case
  • Case and administrative management duties including faxing scripts and requisitions, completing follow-ups, responding to text-based inquiries, and monitoring queues to ensure timely response to all requests
  • The working hours for this position are Sunday to Thursday from 12:00PM to 8:00PM

Skills and Experience:

  • Registered Practical Nurse or Licensed Practical Nurse in good standing with the applicable provincial college (i.e. the College of Nurses of Ontario)
  • 3-5 years of practicing experience
  • Possess a combination of experience in mental health and/or case management.
  • Excellent written and verbal communication skills in English and French
  • Critical thinker with strong organizational skills
  • Ability to multitask
  • Team player & strong interpersonal skills
  • Quality-driven with a focus on exceptional customer service
