Your role as Care Coordinator (Reduced Scope)
Dialogue is looking for a Care Coordinator – Reduced Scope to join our team and help improve the well-being of our members, while supporting the administrative needs of our healthcare team. In this hybrid role, you will focus on essential tasks such as processing prescriptions, sending referrals, scheduling follow-up appointments, and supporting our members as needed.

What you'll be doing
- Acting as the first point of contact between the patient and the medical team
- Post-appointment follow-up, including sending prescriptions, scheduling follow-up appointments, sharing navigation resources, and sending recommendations by fax
- Liaising between medical support and technical teams to help patients and providers resolve various medical, technical, or logistical issues
- Assisting patients in finding resources in order to deliver a memorable experience
- Committing to ensuring complete customer satisfaction
- Communicating with pharmacies and other healthcare facilities by fax or phone, as needed
- Providing administrative support to clinicians
- Advising patients to help them navigate their provincial healthcare system
- Ensuring efficiency throughout the patient journey in collaboration with members of the care team
We'd love to hear from you if you have
- Personality traits such as autonomy, attention to detail, initiative, and a positive attitude
- Availability to work evenings and weekends, as work schedules include day and evening shifts (from 6:00 a.m. to midnight, seven days a week). Our operational needs are primarily in the evenings and on weekends
- Excellent written and verbal communication skills
- The ability to manage crises and resolve problems quickly while remaining calm
- Professional experience within the public healthcare system
- A desire to work in an environment that encourages autonomy, patience, and empathy
- Motivation to work in a dynamic, patient experience focused environment
- An interest in an innovative work setting
- Comfort with computer tools and technology
- Strong organizational skills and attention to detail
- Good time management skills and the ability to prioritize tasks effectively

- Excellent written and verbal communication skills
